Lidocaine’s Mechanism of Action: The Channel’s State Sets the Affinity
Lidocaine has no single dissociation constant. In cardiac Purkinje fibres the half-blocking concentration ran from over 300 uM with inactivation removed to about 10 uM with inactivation nearly complete. Four decades of work show why that thirty-fold shift is the mechanism, not an artefact.
